Very easy visa extension process. As mentioned before, bring a copie of your passport and the page with your indonesian visa. We didnt have a flight ticket to leave the country and it was no problem, just tell them that you leave indonesia by ferry. They then just wanted to have a copy of our in-flight-ticket. You can make copies in the same building or if you need to print, there is a digital print service next to the building.

Easy and rapid Visa extension process.
I took me 30min the first day to photocopy my passport, fill in the information form, take picture and fingerprints with the officer, and pay the 500k. They then ask you to come back 3 days later to pick up your passport.
Make sure to have clothes that cover knees and shoulders + closed shoes. If needed, you can buy a sarung in a market in front of the mosquee (can be negociated at around 30k).

Very easy process to extend your visa - it only takes 2 visits as long as you bring all the required items as mentioned by other reviewers. All the staff are very friendly and helpful. Note that opening hours are Monday - Thursday 8am - 4pm and Friday 8.30am - 4.30pm with a lunch break from 12 noon - 1pm every day.

Visa extension is relatively easy, no need to hire a agent if you stay in Mataram or nearby for a few days anyway and want to save some money. Staff is friendly and helpful.
Some points should be considered for visa extension:
0. You need to go there 2 times. 2nd time around 3 days later.
1. Dress well; long pants, no flip flops. No entry otherwise.
2. Bring copy of passport (pictures page and entry visa. - Copies can be made at the immigration but no printing.
3. Copy of exit flight ticket. Needed on paper. There is a nearby place to print the ticket. Just ask.
4. Pay the fee at the same day you went there on the first day. Payment can be done at the post office which is around 2km away.
5. Don't lose the recipe of your passport.

Make a passport and visa, but we have a trouble when we will take the passport. I suggest you to try online registration to make passport.

Best Visa Extention Experience! (review is based on my voa extantion)
Notes: no tanktop, no shorts and no flip flops, else you're not allowed to enter.
Bring the papers you need!
Bring 1 passcopy of the arrival stamp with that small payment paper, (revived at the paiment counter for voa)
Bring 1 passcopy of the first page.
Bring copy of flight ticket (departure).
1. Go to the second floor.
2. Say what you need and fill out your formular.
3. Wait (if you come in the morning then your maybe lucky to do the biometric data (photo + fingerprint) at the same visit. // Actually it's done next days I think.
4. Now wait for 3 business days.
5. Come back and pick up your passport.
We spend about 40 minutes in total at this immigration office.
Tipp: Go visit the gili Islands, whyle your waiting for your passport (they 1h drive and 1h boat away) just take a public cab (or my blue bird app) and take the cheep public ferry.
Give me a like for my contribution to maps.

They take a 2 hour lunch break so beware. I don't understand why they wouldn't have people on rotation. We had our tickets but we were just before the cutoff so we ended up being at the office for 4 and a half hours for what should've been a simple visa extension and had to get new tickets.
Also, open toed Birkenstocks appear to be considered shoes but flip flops aren't. They're strict on the dress code so save yourself time and just cover everything you can.
Good thing is they have a printer and photocopy downstairs if you're last minute and forgot to print out a few details...

Generally the immigration staff are quite nice and try to help you as much as possible, much gratitude to Bu Lena (front desk) and everyone there. During Ramadhan one staff told me off for drinking eventhough this is probably not public policy, it's good to just acknowledge others before you drink or eat in public during Ramadhan so as not to cause misunderstanding. You know what its like sometimes living abroad, there are schedule misunderstandings/problems with dates, forgetfulness and so on, they have been supportive of my existence as a monk, so thanks so much for being nice and helpful, I owe you one as they say.

Dress appropriately- no shorts, sleeveless shirts, or flip flops.
I extended my visa here with no issues. I came on 7/13 to fill out the paperwork. The forms are free. Be sure to bring all required documentation (multiple copies of passport, visa stamp, outbound ticket). The form is in Indonesian but they have a sample one filled out in English to help. They took my form/passport and told me to come back on 7/18 for fingerprints/photos in the morning and passport pickup in the afternoon. These instructions are also listed on the receipt they give you. Total cost 355k.

Very convenient visa extension, you only have to be there on two days:
Once on the first day to fill in the forms, twice on the date they tell you, for us it was a week later. In the morning you take pictures and fingerprints and pay, then you return in the afternoon to pick up the passport.
We never waited longer than 20 min for each step.
Just keep in mind to be dressed according to the code and bring all fotocopies (you can get them around the corner but it's expensive).

To extend 30 day VOA:
-Wear - no shorts, no flip flops and do not show shoulders e.g. no vest.
- bring - passport, photo copy of passport picture page, photo copy of passport visa page, print out of flight confirmation out of indonisia. You can get photo copies/print out round the back of the embassy, but it costs 10000 per page! Plan ahead if possible. They also say you need you entry flight confirmation, but we didn't have this and it was ok.
- visit 1 we arrived on tuesday got there about 10am went up 1 floor and go to the desk and asked for a form to extend. Filled out the form leaving sponsor blank. Handed it in with documents listed above. You then sit for about 2 hours and then your name is called and you go back to the desk. You get given a receipt and a date to come back. We got told to come back Friday, but we planned to do rinjani while we waited and said we would come back on Monday instead, which was ok.
- visit 2 we arrived on Monday at 10am. We handed in our receipt at the same desk and sat down for about 2 hours. We where then called up and they took photos, finger prints and signiture. Then you go straight to another desk and pay (355,000 idr in september 2017). Then go to another desk and get told when you need to come back and collect. We got told 3pm same day.
Visit 3 - came back at 2.30pm hoping to get it early. Handed in our receipt and sat down. Waited about 1 hour then received our passport with visa. Check the expiry date on your visa is correct. And you good to go.
Alot of sitting and waiting so bring a book, laptop, phone or something to do.
