I got about 15 sea urchin punctures in my knee while scuba diving. After spending an hour or two googling the problem, i decided to see a doctor to have them removed. I figured the local hospital would be ideal; surely they have people coming in every day with urchin punctures. I figured they'd have tons of experience getting them out.
Nope.
The first doctor i spoke to (maybe a nurse practitioner?) suggested pounding on the knee to break up the spines and speed absorption into the body. Most of the google articles i found had said this was a bad idea--the big danger of urchin spines over a joint is nerve damage and this would have increased my risk of serious injury, not decreased it.
I refused this treatment. The doctor said she would ask her surgeon for alternatives. She returned a few minutes later and told me that her surgeon had suggested flushing the area with ammonia, because urine was a great treatment but they couldn't use urine at a hospital.
At this point, i pretty much gave up. They'd only suggested folk remedies. Every article i'd read had told me NOT to try either pounding or peeing on the punctures.
I asked for antibiotics and got a prescription, so that was good, and then i got the full "westerners pay more" fee of 650k rupiah. Now, i don't mind paying more. The price hike is fair, as far as i'm concerned--so long as i'm receiving quality treatment. In this case, i got very bad, potentially harmful treatment.
I did ask repeatedly about removing the spines one by one, which most of the articles suggested. I had taken a photo of the area right after the dive so i'd have a record of where each one was. Instead, the nurse/doctor suggested making a big x-shaped cut over the whole affected area--not removing the spines one by one, just a big X--and flushing it with hydrogen peroxide. I declined.
Eventually, they tried to remove a couple of spines individually but they were only humoring me & had no experience with the procedure. No use.
In short: If you're thinking of seeing a doctor for a sea urchin wound, i would suggest waiting until you can reach a different establishment. Amazingly, the Labuan Bajo hospital is not equipped to help.

If you can help it, do not go to this hospital if you are non-Indonesian - they will charge you 10x what Indonesians pay for any service/consult
This is a private hospital too, so this policy strictly exists Just to screw over tourists.
It was 650,000 rupiah for my consult
And 55,000 for my Indonesian girlfriends consult (exact same service)
I've been to several hospitals throughout Indonesia, and have not been treated this way. Avoid this place if you can

Pasien keluar dari RS tanpa disertai dengan dokumen diagnosa padahal sudah diberikan tindakan dan CT. Saat ditelepon (saya hendak minta dikirimkan hasil diagnosa), berkali-kali baik operator maupun bagian Customer Service tidak menjawab. Kamar yang ditawarkan pada gambar berbeda jauh dengan kondisi sebenarnya. Saat minta diberikan obat generik, dokternya malah bilang "loh, tapi kan ibu pakai asuransi". Pada saat hendak dipindahkan ke kamar, pasien dimintai deposit uang yang besarnya 10x lipat biaya kamar. Situasi emergency room berisik, bukan karena banyak pasien melainkan staff rumah sakit saling mengobrol dengan suara keras.
